REGISTRATION INCLUDES: 7 HOURS EXHIBITION
Eight-foot skirted table in carpeted area with chair, all seminars and CEUs when requested: All meals provided by MACH and Private Reps Luncheon. Exhibiting Wednesday and Thursday. Setup starts Wednesday, June 25, at 1:00 p.m. to be completed by 4:00 p.m. 

EXHIBIT FEE:
To be received by and no later than May 19, 2008.

EARLY DISCOUNT REGISTRATION:
1 skirted table - $995.00. Deadline May 19, 2008.

AFTER MIDNIGHT MAY 19, 2008:
1 table - $1400.00

CANCELLATION prior to May 23, 2008 is subject to a $300 processing fee. After June 6, NO REFUNDS on cancellations or no shows.

REPS MEETING AND LUNCHEON:
Reps Appreciation Luncheon, Thursday, June 26, 2008.
Please indicate how many reps will attend in order to help us with menu planning. This luncheon is sponsored by MACH to give your company an opportunity to give suggestions on improvements and to hear our plans for the future. It is important that your reps attend. There will also be a drawing for a 2009 complimentary exhibit space during the luncheon.

A reservation card will be in conference packet for each company registered (this will be used for the 2009 drawing). 

SHOW YOUR STUFF CONTEST:
Best exhibit booth contest - Two categories:
   1. Most fun/creative/eye catching booth
   2. Best presentation of new technology booth.

HOSPITALITY SUITES:
Hospitality Suite Reservations must be approved by MACH and made directly through the individual hotel listed. Exhibitors shall not host any other event competing or coinciding with the MACH Conference Agenda.

SECURITY:
MACH provides 24-hour security in exhibit hall.

STUFFING TOTES:
No cost will be charged for stuffing tote bags with manufacturers' handouts. Contact Debby Drewry at 859-283-5404 to sign up and recieve shipping instructions. No stuffers accepted after May 25, 2008.

DISPLAY SHIPPING INFORMATION:
MACH is recommending that Geo Fern Company in Louisville, Kentucky, be used for all exhibit display shipping. Your items will be brought to your exhibit space at the Hyatt Regency on Wednesday, June 25, 2008 and picked up on Thursday, June 26, 2008. You will set up your own display.

Once you have registered with MACH you will automatically receive an information pack from Geo Fern Company 502-367-0254. ELECTRICITY ORDERED THROUGH GEO FERN COMPANY.

SPECIAL NOTICE OF CHANGE FOR 2008
There will be no bingo cards used and no manufacturers' $100 give-a-way. In place of these promotions there will be door prize drawings during each exhibit session. The attendee must be present to win so we can continue to guarantee their attendance during your exhibit time.

We would like to encourage you to give as many small items for the door prize drawings as you can. The larger contributions, such as hearing aids or hearing aid supplies, will be used in the live auction on Wednesday and Thursday nights. 

Dani Land will be contacting manufacturers to accept donations. 

RESERVE TICKETS FOR CHURCHILL DOWNS:
Friday, June 27, 2008 we will attend races at the home of the Kentucky Derby, in Millionaires Row atop Churchill Downs. Tickets include transportation, admission, program, seating and buffet for $25 each. Tickets are limited and early reservations are encouraged.

Please include your exhibit fee or (for earlier reservation) send a separate check with memo showing how many tickets for CHD. Make checks payable to Mid-America Conference on Hearing. Tickets will be placed in reps exhibit pack at conference. 

Buses depart from front door of the Hyatt starting at 1PM. Buses will return to hotel starting at 6PM; last bus runs after final race. (drop off and pick up at same location)

PLEASE ADHERE TO CHURCHILL DOWNS DRESS CODE:
Business Casual (tip: dress like going to a fine restaurant). No jeans or sneakers. All men's shirts must have collar. Refunds will NOT be made for dress code violations.
